Carole Lee Blackburn, 66, of Cody, died at West Park Hospital on April 25, 2015.
She is survived by her husband Randy Blackburn and her sons Tad and Cory Beals.
Memorial services will be on Saturday, May 2, at the Cody Auditorium at 6 p.m.
Cremation has taken place with Ballard Funeral Home.
